Our nurse case managers work with our doctors, pharmacists, counselors and other care providers to ensure that our clients are adhering to their medication regimens, that they are keeping their appointments, and that they understand their doctor's instructions.

Medical adherence case management also identifies and removes barriers to a client's ability to stick to their treatment regimen. If issues such as housing, food or child care are barriers, our nurse case managers will refer them to the public benefits department for assistance.

This service was designed for HIV/AIDS care, but is open to our non-HIV/AIDS clients who have chronic conditions, such as diabetes, hepatitis, heart disease and hypertension. Our prenatal clinic clients can also use it.
