TITLE: Nurse Practitioner

JOB SUMMARY:
Provide primary care, urgent care and HIV specialty care in collaboration with medical staff. Will provide nursing support in emergency situations in collaboration with the Nurse Manager. Follows established standards, procedures and practices, and give specific patient management directions to nursing and other staff in compliance with local laws and regulations.

PRIMARY DUTIES:

  • Provide primary medical care, including medical status assessments, performs diagnostic, therapeutic and clinical procedures.
    • Screening patients to determine the need for medical attention.
    • Taking a patient history.
    • Performing a physical examination.
    • Recording patient data in medical records.
    • Performing the diagnostic, therapeutic and clinical procedures as necessary.
    • Adherence with current established standard of HIV care, primary care and gay, lesbian, bisexual and transgendered care.
    • Documented monitoring of adherence to treatment.
    • Participate in the provision of GYN services.
  • Participates in formal and informal patient teaching activities.  Provide preventive health services, including HIV risk reduction, health promotion and disease prevention
    • Documentation of education provided in patients file.
    • Documentation of assessment of risky behaviors, discussion of consequences of risky behaviors, and referral for risk reduction if necessary.
  • Coordinates patient referrals to case management, mental health and substance abuse services and outside social services and community organizations.
  • Responsible for analysis of clinic needs, maintaining high level of expertise in awareness of new trends and treatments and appropriate program development
  • Participates in various administrative duties as assigned.
  • Provision of Urgent Care
    • Patients requiring urgent care are evaluated within one hour of registration at the Clinic.
    • Rapidly responsive to nursing staff who performs the initial triage.
    • Ensure continuity of care before leaving duty.
    • Performs venous and arterial bloodwork, spinal taps, abdominal taps, arthrosynthesis, I and D of abcess, and other minor surgeries.
  • Maintain accurate patient records, chart documentation and documentation for administrative purposes and reimbursement of services.
  • Recruit and coordinate volunteer staff, as necessary
    • Maintain and record number of hours provided by volunteer NP or PA staff.

PRINCIPAL WORKING RELATIONSHIPS:

  • Reports to medical site director for clinical and administrative supervision.
  • Works closely with the Clinic support staff, other Nurse Practitioners, Physicians, Physician Assistants, Nursing staff and Nutritionist.
  • Works in collaboration with Case Managers, Mental Health/Substance Abuse providers, Social Workers and Dentists.
  • Works with WWC and local Pharmacies regarding drug assistance program.
  • Coordinate care with local hospitals as needed.
REQUIRED SKILLS, KNOWLEDGE & ABILITIES
  • A proven sensitivity and knowledge about the provision of primary medical care.
  • Sensitivity to issues that affect the gay, lesbian, bisexual, transgender and HIV communities.
  • Ability to promote client confidence in treatment relationships with a diverse client population.
  • Job requires a high level of problem-solving due to the complexity of the situations encountered.
  • Excellent written and oral communication skills.
  • Fluent in Spanish preferred.

PHYSICAL REQUIREMENTS:

  • Intermittent physical activity including walking, standing, sitting, lifting and supporting patients.
  • Incumbent will be exposed to virus, disease and infection from patients and specimens in working environment.
  • Incumbent may experience traumatic situations

MINIMUM AND DESIRABLE QUALIFICATIONS:

  • Licensed nurse practitioner in Virginia
